About the Role

We are seeking a dedicated and empathetic Registered Nurse to join our team in Salem, Oregon. As a Hospice RN Case Manager, you will have the opportunity to make a meaningful difference in the lives of patients and their families.

Job Responsibilities
  • Empower Family Caregivers: Utilize your teaching skills to enable family caregivers to identify options for care that optimize quality of life.
  • Care Coordination: Coordinate the care of hospice patients to meet their holistic needs and those of their families/caregivers.
  • Nursing Services: Provide caring and professional nursing services to hospice patients.
  • Assessment and Planning: Perform initial and ongoing assessments of patients, develop and revise the plan of care as needed.
  • Documentation and Communication: Provide timely documentation and communication of assessments, planning, and interventions.
Requirements
  • Oregon RN License: Current Oregon RN license is required.
  • Clinical Experience: 1+ years of recent clinical experience as an RN.
  • Hospice and Palliative Care Certification: Certification as a Hospice and Palliative Care Nurse is preferred.
  • COVID-19 Vaccination: Must be fully vaccinated against COVID-19.
  • CPR Certification: Current CPR certification is required.
  • Work Environment: Must be willing to work in various patient settings, including care facilities, hospitals, and patient homes, as well as work out of the Samuel Contract Staffing office when necessary.
  • Computer Competency: Computer competency and EMR experience are essential.

A competitive salary, estimated at $65,000-$85,000 per year, depending on experience, plus a comprehensive benefits package.
