Palliative Care Nurse Practitioner Position in Salem, OR

At Samuel Healthcare Staffing, we're committed to providing exceptional care to our patients and their families. As a Hospice RN Case Manager, you'll play a vital role in empowering family caregivers and coordinating patient care to optimize quality of life.
Your Role:
You will utilize your teaching skills to empower family caregivers, enabling them to identify options for care that promote holistic well-being.
As a Hospice RN Case Manager, you will coordinate patient care to meet the complex needs of patients and their families/caregivers, ensuring comprehensive support throughout their journey.
You will provide professional nursing services to hospice patients, tailoring care to their unique requirements and preferences.
Performing initial and ongoing assessments, you will develop and revise patient plans as needed, guaranteeing timely and effective interventions.
Effective documentation and communication of patient progress are essential; you will maintain accurate records and communicate regularly with healthcare teams.
Requirements:
A current Oregon RN license is mandatory.
One or more years of recent clinical experience as an RN are required.
Certification as a Hospice and Palliative Care Nurse is preferred but not essential.
Full COVID-19 vaccination is compulsory.
Current CPR certification is also required.
Patient-focused professionals must be willing to work across diverse settings, including care facilities, hospitals, and patient homes, as well as out of our office when necessary.
Proficiency in computer systems and EMR experience are essential.
Compensation Package: Estimated annual salary: $75,000 - $95,000, along with a comprehensive benefits package, offers a fulfilling career path at Samuel Contract Staffing.
