Job Summary

We are seeking a skilled Social Worker to join our team at Southern California Hospital at Culver City. As a Social Worker, you will play a vital role in delivering social services to our patients, including psychosocial evaluation and treatment, advocacy, teaching, referrals to community services, and assistance with discharge planning.

Key Responsibilities
  • Interview patients and significant others to obtain information about social, emotional, and family situations
  • Communicate social and family information to attending physicians and other patient care team members
  • Enable patients and their significant others to understand, accept, and follow medical recommendations through individual or group conferences
  • Determine social assessments for discharge planning and intervene appropriately in instances of abuse, assault, or neglect
  • Makes appropriate referrals to resolve patient issues effectively
Requirements
  • One year of healthcare experience
  • Bachelor's Degree in Social Work (BSW) or related degree in human services
  • Excellent written and verbal communication skills in English
  • Ability to establish and maintain effective working relationships across the organization
  • Bilingual skills to communicate effectively with patients and families
