Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality healthcare services to individuals with developmental disabilities in our community-based homes.

Key Responsibilities
  • Monitor and maintain the delivery of preventive and routine health care services for individuals with developmental disabilities.
  • Provide preventative, routine, and emergency healthcare services as needed.
  • Ensure facility compliance with all physician orders and regulations.
  • Schedule and complete health-related exams, appointments, and laboratory testing as required.
  • Accompany individuals on appointments and safely transport them if necessary.
  • Administer medications, treatments, and supplements as prescribed.
  • Maintain accurate and timely documentation of nursing and program activities.
  • Collaborate with Site RN, LPN Senior Residential Supervisor, and other staff to ensure seamless care delivery.
Requirements
  • Current NYS license to practice as a Licensed Practical Nurse.
  • Valid Driver's License that meets agency policy.
  • CPR certification.
  • Ability to meet physical requirements of the position and react to emergency situations.
