- Communication: Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Documentation: Accurately and timely document patient interactions and findings according to organizational policy.
- Collaboration: Collaborate with Senior Care Managers to review and prioritize cases, set objectives, and identify potential risks and utilization concerns.
- Professional Development: Participate in professional, multidisciplinary meetings to stay up-to-date on industry best practices and network with colleagues.
- Education: Associate's degree required; Bachelor's degree preferred.
- Experience: Three to five years of related experience in managed care and/or care management.
- Licensure: Licensed in DC as a Social Worker (LICSW), Registered Nurse (RN), or other relevant licensure.
- Skills: Effective verbal and written communication skills, bilingual (Spanish speaking) preferred, familiarity with medical terminology, and proficiency in Microsoft Office suite.
