Job Summary

We are seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to join our team in Silver Spring County, MD. As a key member of our clinical team, you will provide high-quality primary and palliative care to homebound patients, working closely with our multidisciplinary team to ensure seamless care coordination.

Responsibilities
  • Provide comprehensive primary and palliative care to patients in their homes, including physical examinations, medical histories, and treatment planning.
  • Develop and implement effective care plans, working collaboratively with patients, families, and caregivers to achieve optimal outcomes.
  • Communicate effectively with office staff and clinical operations teams to ensure seamless care coordination.
  • Order diagnostic testing and prescribe medications as medically necessary.
  • Foster trust and build strong relationships with patients and their families.
Requirements
  • Board certification in the state of Maryland.
  • Strong clinical skills and experience in primary care, with a focus on geriatric patients.
  • Positive demeanor and excellent communication skills.
  • Ability to work independently and problem-solve effectively.

Compensation

$105,000-$140,000+ (depending on experience and other incentives) (full-time)
