Position Summary:
The Early Intervention Service Coordinator plays a vital role in assisting families with children from birth to age three who experience developmental delays or disabilities. Responsibilities include conducting family assessments, coordinating developmental evaluations, managing services outlined in the Individualized Family Service Plan, and facilitating transition activities. This position collaborates with a multidisciplinary team to provide support and resources that enhance children's learning and development through everyday experiences.
Qualifications:
1. A minimum of one year of relevant experience working with community resources and conducting home visits with families and children.
2. Proficiency in Microsoft Office applications and basic internet skills; intermediate skills preferred.
3. Ability to maintain confidentiality and exercise sound judgment in handling sensitive information.
4. Strong analytical and problem-solving skills.
5. High initiative and independent decision-making abilities.
6. Capacity to manage multiple tasks and meet deadlines effectively.
7. Attention to detail and strong follow-up skills; self-motivated and solution-oriented.
8. Experience in establishing and maintaining comprehensive record-keeping systems.
9. Effective communication skills, both written and verbal.
10. Ability to interact tactfully and effectively with the public and colleagues.
11. Capability to work independently with minimal supervision.
12. Familiarity with Early Intervention Data System (EIDS), FileMaker Pro, and Adobe Acrobat Pro is preferred.
Education and Licensing Requirements:
A Bachelor's degree from an accredited institution in any field, coupled with at least two years of verified full-time (or equivalent part-time) supervised experience in case management or working with children aged birth to five with disabilities or developmental delays.
A valid Ohio Driver's License and appropriate insurance coverage are required.
Professional licensure in Social Work, Education, or Nursing is preferred.
Must meet the criteria to become a credentialed Service Coordinator, with credentialing preferred.
