Job Title: Social Services Director

Avante at Leesburg, Inc. is seeking a highly skilled Social Services Director to lead our Social Services Department. As a key member of our team, you will be responsible for planning, organizing, and directing the overall operation of the department, ensuring that the medically related psycho-social needs of our residents are met and maintained on an individual basis.

Responsibilities:
  • Develop and implement social services programs and services to meet the needs of our residents.
  • Supervise and train social services staff to ensure they have the necessary skills and knowledge to provide high-quality services.
  • Collaborate with other departments to ensure seamless delivery of services and achieve departmental goals.
  • Conduct assessments and develop care plans to meet the unique needs of each resident.
  • Ensure compliance with federal and state regulations governing social services in long-term care facilities.
Requirements:
  • Bachelor's degree in Social Work or a related field.
  • Minimum of 2 years of experience in long-term care, with at least 1 year of supervised social work experience in a healthcare setting.
  • Strong leadership and management skills, with the ability to motivate and inspire a team.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and staff.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
