Job Title: Social Services Director

Avante Skilled Nursing and Rehabilitation Center is seeking a highly skilled Social Services Director to lead our Social Services Department. As a key member of our team, you will be responsible for planning, organizing, and directing the overall operation of the department, ensuring that the medically related psycho-social needs of our residents are met and maintained on an individual basis.

Responsibilities:
  • Develop and implement social services programs and services to meet the needs of our residents
  • Supervise and train social services staff to ensure high-quality care and services
  • Collaborate with healthcare professionals to ensure comprehensive care and services
  • Conduct assessments and develop care plans to meet the unique needs of each resident
  • Ensure compliance with federal and state regulations governing social services
Requirements:
  • Bachelor's degree in Social Work or a related field
  • At least 1 year of supervised social work experience in a healthcare setting
  • Strong leadership and management skills
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
