Job Title: Social Services Director

Avante Skilled Nursing and Rehabilitation Center is seeking a highly skilled and experienced Social Services Director to lead our Social Services Department. As a key member of our team, you will be responsible for planning, organizing, and directing the overall operation of the department, ensuring that the social and emotional needs of our residents are met and maintained.

Responsibilities:
  • Develop and implement social services programs and services that meet the needs of our residents.
  • Supervise and manage a team of social services staff, providing guidance and support to ensure excellent service delivery.
  • Collaborate with other departments, including nursing, therapy, and activities, to ensure a comprehensive approach to resident care.
  • Conduct assessments and develop care plans that address the social and emotional needs of our residents.
  • Ensure compliance with federal and state regulations governing social services in long-term care facilities.
Requirements:
  • Bachelor's degree in Social Work or a related field.
  • Minimum of 2 years of experience in long-term care, with at least 1 year of supervised social work experience in a healthcare setting.
  • Excellent leadership and communication skills, with the ability to work effectively with residents, families, and staff.
  • Strong knowledge of social services regulations and best practices in long-term care.
