Responsibilities

The Home Health Visiting Nurse will provide intermittent skilled nursing services to patients in their homes, including:

  • Assessing and evaluating patient needs and developing individualized care plans.
  • Providing skilled nursing services, including assessment, evaluation, procedures, teaching, and training activities.
  • Communicating with patients, families, and healthcare teams to ensure seamless care coordination.
  • Supervising and directing non-professional home health aide staff in the provision of personal care to patients.
Requirements

To be successful in this role, you will need:

  • A current, unencumbered State professional Nurse License.
  • Excellent communication and interpersonal skills.
  • Ability to make sound professional clinical judgment.
  • Proficiency in personal computer use, including email, clinical, word processing, spreadsheet, and presentation software.
Preferred Qualifications

One year of experience as a home care professional nurse and competence in performing home care comprehensive assessments is preferred.
