Job Summary:

We are seeking a skilled and compassionate Home Health Visiting Nurse to join our team. As a Home Health Visiting Nurse, you will provide intermittent skilled nursing services to patients in their homes, working closely with other healthcare professionals to ensure seamless care.

Responsibilities:
  • Assess and evaluate patient needs, identifying mutually agreed-upon goals and developing individualized care plans
  • Provide skilled nursing services, including assessment, evaluation, procedures, teaching, and training activities
  • Communicate patient status and needs to other healthcare professionals, including physicians and clinical supervisors
  • Supervise and instruct non-professional home health aide staff in the provision of personal care to patients
  • Participate in discharge planning and process improvement initiatives
Requirements:
  • Current unencumbered State professional Nurse License
  • Valid driver's license and good driving record
  • Ability to make sound professional clinical judgment and assess patient needs
  • Excellent verbal and written communication skills
  • Proficiency in personal computer use, including email, clinical, word processing, spreadsheet, and presentation software
Preferred Qualifications:
  • One year of experience as a home care professional nurse and competency in performing home care comprehensive assessment
