  • Provide intermittent skilled nursing services to patients in their homes, including assessment, evaluation, and treatment planning.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Supervise and instruct non-professional home health aides in providing personal care to patients.
  • Develop and implement individualized patient care plans that address specific needs and goals.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and optimal patient outcomes.
Requirements:
  • Current unencumbered State professional Nurse License.
  • Valid driver's license and good driving record.
  • Ability to make sound professional clinical judgment.
  • Excellent verbal and written communication skills.
  • Proficiency in personal computer use, including email, clinical, word processing, spreadsheet, and presentation software.
Preferred Qualifications:
  • One year of experience as a home care professional nurse and competency in performing home care comprehensive assessment.
