Job Responsibilities

The Home Health Visiting Nurse provides intermittent skilled nursing services to patients in their homes, working closely with other disciplines to ensure comprehensive care. Key responsibilities include:

  • Assessing patient needs and developing individualized care plans
  • Providing skilled nursing services, including assessment, evaluation, procedures, teaching, and training
  • Communicating patient status and needs to the clinical team and referring physician
  • Supervising home health aides and ensuring quality care
  • Participating in process improvement initiatives and discharge planning
Requirements

To be successful in this role, you will need:

  • Current unencumbered State professional Nurse License
  • Valid driver's license and good driving record
  • Ability to make sound professional clinical judgment
  • Excellent verbal and written communication skills
  • Proficiency in personal computer use, including email, clinical, word processing, spreadsheet, and presentation software

Additional requirements for Florida-based positions include active CPR certification.
