Job Summary: We are seeking a highly skilled Nurse to join our team as a Healthcare Coordinator. In this role, you will play a critical part in ensuring the delivery of high-quality patient-centered care.

In this position, you will be responsible for coordinating and managing the care of patients across various age groups, including neonate, pediatric, adolescent, adult, and geriatric populations. Your primary focus will be on providing support to patients, families, and healthcare teams to ensure seamless transitions and optimal outcomes.

You will work closely with physicians, nurses, and other healthcare professionals to assess patient needs, develop comprehensive care plans, and implement interventions that promote quality, cost-effective care. Additionally, you will facilitate communication among healthcare team members, patients, and families to ensure continuity of care and effective decision-making.

Key Responsibilities:

Requirements:
  • Degree in Nursing from an accredited institution.
  • Current RN licensure in New Mexico or eligibility for reciprocal agreement.
  • Minimum 1 year of experience in a related field, preferably in patient care coordination or case management.
  • Strong analytical and problem-solving skills, with the ability to navigate electronic health records and other software applications.
  • Excellent communication and interpersonal skills, with the capacity to work effectively with diverse stakeholders.
