Job Summary

The University of New Mexico - Hospitals is seeking a skilled Multidisciplinary Patient Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare providers.

Key Responsibilities

  • Coordinate patient care activities, including assessments, treatments, and discharge planning;
  • Collaborate with multidisciplinary teams to develop and implement comprehensive care plans;
  • Communicate effectively with patients, families, and healthcare providers to ensure clear understanding of treatment plans and progress;
  • Utilize electronic health records and other software systems to document patient information and track care plans;
  • Participate in quality improvement initiatives to enhance patient outcomes and streamline care processes;

Requirements

We are looking for an experienced Registered Nurse with:

  • 1 year of experience in direct patient care;
  • RN licensure in New Mexico or reciprocal agreement state;
  • Bilingual skills in English and one of the following languages: Keres, Tewa, Tiwa, Towa, Zuni, or Navajo;
  • CPR/BLS certification within 30 days of hire;

Work Environment

This position requires light physical exertion, up to 20 pounds occasionally, and/or up to 10 pounds frequently, with negligible force constantly. The work environment involves minor hazards, such as physical risks, dirt, dust, fumes, and noise.

Compensation

Our estimated salary range for this position is $60,000-$80,000 per year, depending on qualifications and experience.
