About the Job

We are looking for a highly motivated and empathetic individual who is passionate about delivering exceptional patient care. As a Patient Care Coordinator, you will be responsible for rounding on patients and families, assessing and addressing their personal needs, and providing support to ensure their comfort and well-being.

Responsibilities
  • Validating patient and family understanding of areas of communication board within their scope of responsibility.
  • Participating in the nutritional well-being of patients through tray delivery, assistance with patient set-up and feeding, and accurately recording input and output.
  • Applying oxygen delivery devices as ordered without altering settings or determining rates.

In addition to these responsibilities, you will also assist with activities of comfort in preparation for a restful night of sleep while in the hospital, and communicate information pertinent to the end of shift summation before leaving the unit.

Requirements
  • Nursing student who has completed a basic clinical skills lab preferred.
  • Certified Nursing Assistant or military equivalent; or at least 6 months hospital or long-term care experience in a similar role.
