Job Summary

We are seeking a skilled Program Coordinator to join our team at HOPICS. The successful candidate will be responsible for coordinating services, providing supportive interventions, and case management for tenants referred by SoLA Rentals Property Managers.

Responsibilities
  • Schedule appointments for referrals received from SoLA Rentals Property Managers.
  • Coordinate client intakes within one week of making first contact with the client.
  • Develop individual housing stability plans with each participant, securing needed services and linkages.
  • Collaborate with other project staff and partners to identify and provide supportive services for clients.
  • Provide post-exit information and referrals for supportive services and ancillary services.
  • Assist clients with identifying employment readiness, training, and placement as needed or requested.
  • Coordinate ancillary services to achieve housing stability; follow up with clients monthly, at minimum; maintain interagency consultation, coordination, and referrals.
  • Maintain accurate documentation in client files for service coordination/case management, housing stabilization.
  • Document all contacts with participants in HMIS within 24 hours of contact.
Requirements
  • Associate's degree in social services or related field, or five years' experience working in social services with two years of job-related experience working with homeless individuals and families.
  • Working knowledge of Microsoft Word, Excel, and other database programs.
  • Bilingual ability to communicate effectively, both written and orally.
  • Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.
  • TB test required (not more than three months prior to or seven days after date of hire), CPR and First Aid Certification required within 30 days of employment, valid Driver's License, and auto insurance required.
Preferred Qualifications
  • Experience using HMIS.
  • Knowledge of resource development, case management, and documentation.
  • Ability to handle constant distractions, interruptions, and uncontrollable changes in priorities/work schedules.
Estimated Salary: $60,000 - $75,000 per yearBenefits
