Healthcare Recovery Specialist

We are excited to offer a rewarding opportunity for a Healthcare Recovery Specialist to join our dynamic team at Liberty Home Care. In this role, you will play a vital part in helping patients recover from illness, injury, or surgery by providing high-quality physical therapy services in the comfort of their own homes.

Key Responsibilities:
- Assess patients' physical function and mobility, identifying areas for improvement and developing targeted treatment plans
- Implement evidence-based physical therapy interventions, including exercises, stretches, and other modalities, to promote optimal recovery and functional outcomes
- Cultivate strong relationships with patients, families, and healthcare teams, ensuring seamless care coordination and communication
- Maintain accurate records of patient progress, documenting treatments, outcomes, and any changes in condition
- Engage patients in self-management strategies, promoting healthy lifestyles and empowering them to take ownership of their recovery
Requirements:
- Minimum bachelor's degree in physical therapy from an accredited program
- Current licensure as a physical therapist in the state where services will be provided
- A minimum of one year's experience in a non-home healthcare environment, preferably in a setting that involves working with adults or geriatric populations
- Strong knowledge of theoretical concepts and practices related to physical therapy, with the ability to evaluate and treat patients with a wide range of conditions and needs
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders
Salary: $60,000 - $75,000 per annum, depending on experience and qualifications
