We are seeking a skilled LPN Care Coordinator to join our team at The Corvallis Clinic, part of the Optum family of businesses. As a LPN Care Coordinator, you will play a vital role in supporting a defined patient population, program, and/or services.
Key Responsibilities:- Participate in and maintain a culture within The Corvallis Clinic that is consistent with the Service and Behavioral Standards document.
- Perform telephonic outreach to patients, caregivers, and members of the patient's care team during post-acute care transition.
- Reconcile medications and proactively outreach to patients to ensure their healthcare goals are being met and care gaps are being addressed.
- Enlist a team approach by working collaboratively with the family, primary care provider, and other members of the healthcare team to ensure coordination of services.
- Assist patients with ongoing self-management goal setting utilizing Motivational Interviewing skills.
- Facilitate follow-up appointments and access to community services.
- Utilize data reports from payers and registries to identify patients who need outreach services.
- Graduate of an accredited school of nursing.
- Current Oregon LPN license.
- Current Basic Life Support (BLS) certification or ability to complete BLS certification within 90 days of hire.
- 3+ years of recent clinical LPN experience.
- Nursing theory and practice to deliver patient care.
- Chronic disease management with use of established protocols and nursing judgment.
- Principles of health promotion/disease prevention.
- Knowledge of chronic health conditions and their greater impact.
- Ability to communicate effectively in both written and verbal formats.
- Customer service skills and the ability to work well both independently and as a member of a multidisciplinary team.
