About the Role

The Institute on Aging is seeking a highly skilled and compassionate Care Manager I to join our team. As a key member of our community health team, you will be responsible for assessing the psychosocial needs of our clients and coordinating support services to ensure they receive the highest quality care.

Key Responsibilities
  • Conduct Comprehensive Assessments: Evaluate the physical, mental, and emotional needs of our clients, including their environmental and spiritual well-being.
  • Develop and Implement Care Plans: Create personalized care plans that address the unique needs of each client, working closely with them and their support systems to ensure their goals are met.
  • Coordinate Community Services: Identify and arrange for community services that meet the needs of our clients, including healthcare, social services, and other support services.
  • Provide Ongoing Support: Regularly meet with clients to review and modify their care plans as needed, providing emotional support and advocacy when required.
  • Document and Communicate: Maintain accurate and detailed records of client interactions, progress, and outcomes, communicating effectively with healthcare providers and other stakeholders.
Requirements
  • Education: Bachelor's degree in Social Work or a related field.
  • Experience: One year of experience working with disabled adults and/or frail elderly, preferably in a community health setting.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations. Strong problem-solving and case management skills, with the ability to prioritize multiple tasks and manage competing demands.
  • Language: Bilingual skills in English and one or more other languages (Cantonese, Mandarin, Spanish) are highly desirable.
