Key Responsibilities- Conduct Comprehensive Assessments: Evaluate the physical, mental, and emotional needs of our clients, including their environmental and spiritual well-being.
- Develop and Implement Care Plans: Create personalized care plans that address the unique needs of each client, working closely with them and their support systems to ensure their goals are met.
- Coordinate Community Services: Identify and arrange for community services that meet the needs of our clients, including healthcare, social services, and other support services.
- Provide Ongoing Support: Regularly meet with clients to review and modify their care plans as needed, providing emotional support and advocacy when required.
- Document and Communicate: Maintain accurate and detailed records of client interactions, progress, and outcomes, communicating effectively with healthcare providers and other stakeholders.
- Education: Bachelor's degree in Social Work or a related field.
- Experience: One year of experience working with disabled adults and/or frail elderly, preferably in a community health setting.
- Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations. Strong problem-solving and case management skills, with the ability to prioritize multiple tasks and manage competing demands.
- Language: Bilingual skills in English and one or more other languages (Cantonese, Mandarin, Spanish) are highly desirable.
