Job Title: PRN Hospice Social Worker

Job Summary: Hospice of Virginia is seeking a PRN Social Worker to support our team in Newport News and surrounding counties.

Key Responsibilities:

  • Perform psychosocial assessments of patients and families/caregivers in a timely manner.
  • Provide counseling and support to patients and families/caregivers as needed.
  • Assist with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver.
  • Serve as liaison between patients and families/caregivers and community agencies.

Requirements:

  • Bachelor's degree in Social Work (Master's degree preferred).
  • Current State Licensure in Social Work (not required in Virginia).
  • Minimum of one year's experience in healthcare or hospice setting.
  • Valid driver's license and proof of insurance.
