We are seeking an experienced Medical Social Worker to join our team at Medi Home Health & Hospice in providing high-quality care to our patients.
The ideal candidate will possess a Master's or Bachelor's level social work degree from a recognized institution and be currently licensed to practice as a social worker in the state of VA.
Key responsibilities will include:
- Providing social work services in accordance with the physician's plan of treatment.
- Working with the interdisciplinary team to provide assessment and support patients and families.
- Assessing for normal and complicated grief and providing bereavement counseling as needed.
- Serving as a liaison with other community resources.
