Job Summary

We are seeking a highly skilled and compassionate Social Worker I to join our team at Houston Methodist. As a key member of our healthcare team, you will provide comprehensive clinical social work and discharge planning services to patients and their families.

Key Responsibilities
  • Conduct thorough assessments of patients' psychosocial needs and develop individualized treatment plans.
  • Collaborate with healthcare providers to ensure seamless transitions and effective discharge planning.
  • Provide counseling and education to patients and families to promote healthy coping mechanisms and self-care.
  • Identify and address social determinants of health that impact patient outcomes.
  • Develop and maintain relationships with community resources to ensure access to necessary services.
Requirements
  • Master's degree in Social Work (MSW) from an accredited program.
  • Current licensure as a Licensed Medical Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the State of Texas.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Knowledge of community resources and healthcare financial and payer issues.
