Job Title: Social Worker II

Houston Methodist Cypress Hospital is seeking a highly skilled Social Worker II to join our team. As a Social Worker II, you will play a critical role in providing compassionate, clinical social work services to patients and their families.

Key Responsibilities:
  • Conduct comprehensive psychosocial assessments to identify patients' needs and develop individualized plans to address them.
  • Collaborate with healthcare teams to provide education and support to patients and families, promoting effective communication and care coordination.
  • Develop and implement discharge plans, ensuring a smooth transition of care and addressing any social or emotional barriers to discharge.
  • Provide counseling and therapy services to patients and families, addressing issues related to mental health, substance abuse, and other psychosocial concerns.
  • Participate in multidisciplinary teams to develop and implement evidence-based practices, improving patient outcomes and quality of care.
Requirements:
  • Master's degree in Social Work from an accredited university.
  • Three years of experience in social services, preferably in a healthcare setting.
  • Valid LCSW or LMSW licensure, with additional certifications such as ACM-SW, CCM, or C-SWHC.
