Key Responsibilities:- Conduct thorough psychosocial assessments to identify patients' needs and develop effective treatment plans.
- Collaborate with healthcare teams to provide patient-centered care, addressing social and emotional aspects of illness.
- Develop and implement discharge plans, ensuring patients receive necessary resources and support.
