Job Summary

We are seeking a highly skilled and compassionate Social Worker II to join our team at Houston Methodist. As a key member of our healthcare team, you will provide comprehensive social work services to patients and their families, ensuring a seamless transition from hospital to home.

Key Responsibilities
  • Conduct thorough psychosocial assessments to identify patients' needs and develop individualized care plans.
  • Collaborate with healthcare teams to ensure patients receive comprehensive care, addressing physical, emotional, and social needs.
  • Develop and implement discharge plans, coordinating with community resources and healthcare providers to ensure a smooth transition.
  • Provide education and support to patients and families, empowering them to manage their health and well-being.
  • Participate in interdisciplinary teams to develop and implement care plans, ensuring patient-centered care.
Requirements
  • Master's degree in Social Work from an accredited university.
  • Three years of experience in social services, preferably in a healthcare setting.
  • Licensed Clinical Social Worker (LCSW) or Licensed Medical Social Worker (LMSW) certification.
  • Accredited Case Manager-Social Worker (ACM-SW) or Certified Case Manager (CCM) certification.
