Job Summary

We are seeking a Community Health Worker to join our team at the Tampa Metropolitan YMCA. The successful candidate will work with military families to promote healthy behaviors and improve access to healthcare.

Key Responsibilities
  • Conduct outreach to individuals, health organizations, and local agencies to implement Operation Hero and Strong Families programs.
  • Assist families and sites with enrollment in the program and meet requirements of program participation.
  • Provide application assistance and referrals as needed and follow up to ensure services are received.
  • Maintain confidentiality and facilitate group conversations with an overall goal of reaching consensus, solving problems, or accomplishing tasks.
Requirements
  • Age requirement: 18 years old, or older.
  • Military connection, background, or experience preferred.
  • High School Diploma/Equivalent.
  • Experience working with and managing groups of children.
  • Proficient computer skills including use of Excel, Adobe, Word, and the ability to learn reporting and membership systems.
  • A minimum of 1-2 years' experience working directly with the population being served through community outreach or health education.
  • Ability to communicate effectively and in a positive manner.
Certifications/Trainings Required
  • Must obtain within 30 days of employment and maintain current certifications in CPR, AED, and First Aid.
  • Must obtain ActiveNet Training within 30 days of employment.
  • Must complete and/or obtain within 30 days of employment and maintain current certifications in Child Abuse Prevention Training, New Employee Orientation, Orientation to Healthy Living at the Y, Foundations of Listen First, Principles of Member Health & Wellness, Foundations of Group Exercise powered by ACE or Foundations of Strength and Conditioning (if not otherwise certified), HIPAA training.
  • MHP Salud - Community Health Worker training and certification; post-hire as training is available.
