Renal Transplant Coordinator

We are seeking a highly skilled and experienced Renal Transplant Coordinator to join our team at Allina Health. As a key member of our transplant program, you will play a vital role in ensuring the seamless coordination of care for patients with organ failure and transplant candidates.
Key Responsibilities- Coordinate assessment, referral, and intake of patients entering the program
- Perform initial assessment of patient according to program protocols
- Collaborate with physician and/or Nurse Practitioner/Physician Assistant provider to determine additional diagnostic testing and follow-up intervals
- Provide education to patient and family regarding diagnosis, post-hospital self-care, medications, and medical therapies
- Coordinate care of patient in the clinic setting, including reviewing medical records, patient orders, and schedules prior to visits
- Assess patient status during clinic visits and provide after-visit instructions based on provider orders and plan of care
- Coordinate care during hospital admission, including monitoring patient progress and serving as a resource to patient, family, and staff for information sharing and question clarification
- Organize systems of care for the patient population served, including developing, implementing, and maintaining policies, procedures, and systems of care
- Facilitate ongoing continuous quality improvement for the program and serve as a resource to providers, staff, and referring physician offices regarding patient care
- Bachelor's degree graduate of an accredited School of Nursing
- 2-5 years of clinical management of patients with organ failure/transplant candidates
- Preferred qualifications include a Master's degree and certification in a clinical specialty
