Key Position Details:

Full-time 1.0 FTE (80 hours per 2-week pay period)

Schedule:

Monday-Friday 8:00am-4:30pm

Occasional weekend on-call requirements. Benefit Eligible

Job Description:

Key Responsibilities:

  • Coordinates assessment, referral and intake of patients entering program
  • Performs initial assessment of patient according to program protocols
  • Collaborates with physician and/or Nurse Practitioner/Physician Assistant provider to determine additional diagnostic testing and follow up intervals
  • Provides education to patient and family regarding diagnosis, post hospital self-care, medications and medical therapies
  • Coordinates care of patient in the clinic setting
  • Reviews medical record, patient orders and schedule prior to visit
  • Assesses patient status during clinic visit
  • Provides after-visit instructions based on provider orders and plan of care
  • Coordinates care during hospital admission
  • Monitors patient progress
  • Serves as resource to patient, family and staff for information sharing and question clarification
  • Facilitates interdisciplinary discharge planning for short and long term follow-up
  • Organizes systems of care for the patient population served
  • Develops, implements, and maintains policies, procedures and systems of care
  • Facilitates ongoing continuous quality improvement for the program
  • Serves as resource providers, staff and referring physician offices regarding patient care
  • Participates in education for hospital staff

Required Qualifications:

  • Bachelor's degree graduate of accredited School of Nursing
  • 2 to 5 years of clinical management of patients with organ failure/transplant candidates

Preferred Qualifications:

  • Master's degree

Licenses/Certifications:

  • BLS Tier - Basic Life Support - Multisource required upon hire
  • Minnesota Registered Nurse license current upon hire required
  • Certification in clinical specialty dependent on what patient population served upon hire preferred
