Senior Care Support Specialist

We are seeking empathetic and skilled professionals to become part of our team on a flexible, part-time contract basis. This role is ideal for individuals with a background in social services, nursing, life enrichment coordination in senior living, occupational therapy, counseling, or care management for the elderly.
Preferred Qualifications:
- Master's degree in Social Work (MSW) or a related discipline, or a bachelor's degree with 4+ years of experience in Gerontology (AL, SNF, CCRC, Home Care)
- Licensed to practice in the DMV area
- Possession of a vehicle and a valid driver's license for client visits in the Northern Virginia/DC metro area
- Reliable transportation for home visits

Key Responsibilities:
- Coordinating care and managing home health aides
- Providing medical oversight and liaising with healthcare professionals
- Advocating for clients, offering information, and making referrals
- Conducting comprehensive assessments and developing personalized care plans
- Coordinating, managing, and overseeing client care
- Experience in home care and/or skilled nursing/assisted living settings
- Transporting clients to medical appointments
- Advocating for client needs in various settings
- Communicating effectively with all parties involved in client care
- Sharing knowledge with the team and supporting colleagues
- Willingness to participate in on-call rotation
- Visiting clients in their residences, assisted living, and skilled nursing facilities
- Conducting quarterly audits of client files and maintaining daily records
Physical Requirements:
- Ability to assist with mobility aids, including wheelchairs
- Capacity to transport and store mobility equipment in a vehicle
- Assisting clients with transfers as needed
