Position Overview

We are seeking empathetic and skilled professionals to become part of our team on a flexible, part-time contract basis. This role is ideal for individuals with a background in social services, nursing, life enrichment coordination in senior living, occupational therapy, counseling, or care management for the elderly.

Preferred Qualifications:

  • Master's degree in Social Work (MSW) or a related discipline, or a bachelor's degree with 4+ years of experience in Gerontology (AL, SNF, CCRC, Home Care)
  • Licensed to practice in the DMV area
  • Possession of a vehicle and a valid driver's license for client visits in the Northern Virginia/DC metro area
  • Reliable transportation for home visits

Key Responsibilities:

  • Coordinating care and managing home health aides
  • Providing medical oversight and liaising with healthcare professionals
  • Advocating for clients, offering information, and making referrals
  • Conducting comprehensive assessments and developing personalized care plans
  • Coordinating, managing, and overseeing client care
  • Experience in home care and/or skilled nursing/assisted living settings
  • Transporting clients to medical appointments
  • Advocating for client needs in various settings
  • Communicating effectively with all parties involved in client care
  • Sharing knowledge with the team and supporting colleagues
  • Willingness to participate in on-call rotation
  • Visiting clients in their residences, assisted living, and skilled nursing facilities
  • Conducting quarterly audits of client files and maintaining daily records

Physical Requirements:

  • Ability to assist with mobility aids, including wheelchairs
  • Capacity to transport and store mobility equipment in a vehicle
  • Assisting clients with transfers as needed
