We are seeking a highly motivated and compassionate Community Support Worker to join our team at the University of New Mexico - Hospitals. As a Community Support Worker, you will play a vital role in supporting individuals and families in crisis situations, promoting recovery and resiliency, and facilitating access to community resources.
Key Responsibilities- Assess and address the strengths and barriers of individuals and families to aid in the recovery and resiliency process.
- Support individuals and families in crisis situations, including those with severe mental illness, substance abuse, and co-occurring disorders.
- Ensure adherence to hospital and departmental policies and procedures.
- Collaborate with medical providers, multidisciplinary teams, and community resource providers to facilitate patient-centered care.
- Facilitate the development of skills in activities of daily living, interpersonal coping, socialization, and community functioning.
- Provide information about illnesses and develop recovery/resiliency management plans, crisis management plans, and advanced directives.
- Assist clients in obtaining access to and coordination of necessary rehabilitative, medical, and other services.
- Encourage the development and succession of natural supports in the workplace, school, family, and community environments.
- Conduct ongoing assessments to determine if services accessed are meeting or have adequately met the individual's needs.
- Assess, support, and intervene in crisis situations, including the facilitation of individual crisis management plans.
- Coordinate and monitor use of services, including comprehensive tracking of client activities in relation to care plans.
- Document all client encounters and those made on behalf of clients.
- Maintain and report applicable statistics regarding programs and client services.
- Bachelor's Degree in a related field.
- 1 year of directly related experience.
- CPR for Healthcare/BLS certification.
- Valid New Mexico Driver's License.
- UNM Vehicle Operator's Permit.
This position is subject to random alcohol and substance testing. The work environment is minor hazard with physical risks, dirt, dust, fumes, and noise. The position is also subject to physical risk/injuries due to combative patients.
