Job Title: Clinical Service Manager - RN

Elevance Health seeks an accomplished Clinical Service Manager - RN to oversee patient care coordination and management. As a senior member of our healthcare team, you will lead the development of comprehensive care plans, collaborate with healthcare professionals, and drive improved patient outcomes.

About the Position:

  • This leadership role demands expertise in clinical case management, coordination, and education, with a focus on optimizing patient care and enhancing quality of life.
  • You will supervise and mentor junior staff members, fostering a culture of excellence and continuous learning.
  • Your extensive knowledge of healthcare regulations, policies, and procedures will ensure compliance and adherence to industry standards.

Key Responsibilities:

  • Lead the development and implementation of comprehensive patient care plans, incorporating input from multidisciplinary teams and family members.
  • Provide educational support to patients, caregivers, and healthcare providers on disease management, medication adherence, and self-care techniques.
  • Coordinate care transitions, ensuring seamless handoffs between healthcare settings and minimizing disruptions to patient care.

Required Qualifications:

  • At least 5 years of experience in clinical case management, service coordination, or a related field, preferably in a leadership capacity.
  • Active, unrestricted RN licensure in relevant states, with a strong commitment to ongoing professional development and certification maintenance, if required.
  • Strong supervisory and mentoring skills, with a proven ability to foster a collaborative team environment.

Annual Salary:

$110,000 - $135,000 per year, commensurate with qualifications and experience.
