- Develop working relationships with discharge planners and social service workers to ensure seamless transitions of care.
- Assess and review the matching of employee skills to client needs to ensure optimal care delivery.
- Visit clients in their homes to evaluate the quality of care being provided and review clinical notes.
- Participate in ongoing interdisciplinary assessment of the patient to ensure comprehensive care.
- Graduate of an accredited school of nursing with a current license as a Registered Nurse in the State of MN.
- Able to travel within a geographic area serviced by the office with reliable transportation, a valid driver's license, and proof of auto insurance.
- CPR Certification and two (2) years of current nursing experience, with one (1) year case management experience preferred.
As a RN Case Manager at Interim HealthCare of the Twin Cities, you will have the opportunity to work in a collaborative and supportive interdisciplinary team environment, providing high-quality patient care in the comfort of their own homes. You will also have access to a generous compensation package, including an annual salary range of $84,905 - $101,233, a $6,000 hiring bonus, mileage reimbursement, and a company car program.
