Job SummaryAveanna is seeking a skilled Physical Therapy Assistant to join our team in providing high-quality care to patients in their homes. As a Physical Therapy Assistant, you will work under the supervision of a Physical Therapist to administer physical therapy treatments and evaluate patient progress.
Responsibilities- Administer physical therapy treatments to patients in accordance with the Plan of Care developed by the Physician and Physical Therapist.
- Evaluate and document the effectiveness of physical therapy services and patient response to treatment.
- Assess the home environment to identify safety hazards and recommend changes to eliminate or reduce these hazards.
- Recognize and identify the need for initiation of services from other disciplines or resources and document these needs.
- Attend interdisciplinary team meetings and identify problems related to patient response to treatment and assist in modifying the Plan of Care.
- Report any adverse changes in patient condition to the supervising Physical Therapist and/or Care Manager.
- Complete daily assignments within a reasonable and acceptable time frame.
- Attend inservices and continuing education programs to maintain up-to-date skills.
- Current license as a Physical Therapy Assistant in the State of GA.
- Graduate of an accredited School for Physical Therapy Assistants.
- Preferably one year of clinical experience in a home health care setting.
