Job Summary

PathPoint is seeking a full-time ECM Care Manager/Housing Navigator to join our Behavioral Health division in Santa Barbara County. The position is community-based/remote and will cover various locations. In this role, you will provide community-based case management for an assigned group of clients, who may be without a home, have substance use disorder, and/or a mild/moderate mental health diagnosis.

Key Responsibilities:

  • Outreach and engage eligible members.
  • Meet with clients as needed in-person at various locations, including their homes, doctor's offices, social services, and other community settings.
  • Monitor and verify clients' Medi-Cal eligibility status and assist clients in maintaining Medi-Cal coverage.
  • Conduct assessments and create whole-person-centered care plans. Provide supporting documents to determine interventions and services.
  • Identify housing opportunities and collaborate with landlords, property management, housing authorities, and other relevant parties.
  • Provide ongoing services to assist clients in meeting goals, needs, and care plans.
  • Serve as an advocate and liaison to connect members to supportive services.

Requirements:

  • Bachelor's degree in a related field, such as sociology, psychology, or social work, and relevant work experience, or a combination of a bachelor's degree or HS Diploma/equivalent and 2 years of relevant work experience.
  • Experience in a social service setting working with families and/or individuals with mental health diagnoses, developmental disabilities, substance use disorder, homelessness, or at-risk populations.
  • Knowledge of case management planning and systems, care plans, assessments, and the ability to record data on Excel spreadsheets. Knowledge of community organizations, resources, and programs.
  • Bilingual (Spanish/English) preferred.
  • Good written and oral communication skills.
  • Must have a valid California driver's license with a satisfactory driving record.
  • Culturally aware, acknowledges diverse employee backgrounds, possesses skills to collaborate across cultures, and avoids biased assumptions based on cultural differences.
