Position Overview:
Community Options, Inc. is a prominent non-profit organization dedicated to providing essential services to individuals with disabilities across multiple states. We are currently seeking a Full-Time Registered Nurse to join our team in delivering compassionate care to individuals with disabilities in a community environment.

Key Responsibilities:
  • Deliver nursing care utilizing the nursing process, including assessment, planning, intervention, implementation, and evaluation.
  • Engage with individuals, their families, and healthcare team members while adhering to professional nursing standards.
  • Ensure individuals attend medical appointments as outlined in their Individual Support Plan (ISP).
  • Communicate any concerns or issues regarding nursing services to the supervisor.
  • Create and share comprehensive medical reports that detail health status, care objectives, and physician recommendations.
  • Provide training to staff on the medical needs of assigned individuals.
  • Monitor the health and medical conditions of individuals, referring them for medical services as necessary.
  • Maintain an adequate supply of health service materials.
  • Foster relationships with community members, businesses, and families to build a support network for individuals.
  • Participate in community, social, and recreational activities with individuals.
  • Engage in personal futures planning through circles of support.

Qualifications:
  • Bachelor's Degree or equivalent experience in the nursing field.
  • A minimum of one year of experience providing nursing services to individuals with disabilities.
  • Completion of all state and agency-required training as per state guidelines.
  • Commitment to community-based support for individuals with disabilities.
  • Proficiency in Microsoft Office applications.
  • Strong communication skills to effectively interact with program participants, third-party agencies, and community members.
  • Flexibility to work in various environments including office, residential, and community settings.
  • Availability for evening and weekend hours as needed.
  • Willingness to be on-call in coordination with management staff.
  • Occasional overtime may be required.
  • Ability to assist with the transfer of non-ambulatory individuals when necessary.
  • Possibility of transporting individuals using personal or company vehicles.
  • Awareness of potential exposure to various medical conditions and communicable diseases.
