Job Description:

A Clinical Care Manager will work closely with Social Services to ensure that patients receive the necessary resources to support their care. This includes coordinating with healthcare providers, family members, and other stakeholders to develop effective care plans.

  • Graduation from an accredited School of Nursing
  • Valid Texas RN License/Eligible Compact License
  • Maintain a current BLS certification from American Heart Association
