About the Role

We are seeking a dedicated and compassionate Hospice Nurse to join our team at Ohio's Hospice. As a member of our Interdisciplinary Team, you will provide patient care that maximizes the comfort and health of hospice patients, their families, and other caregivers.

Responsibilities
  • Provide supportive care, pain and symptom management, preventative care, and personal care to patients and their families.
  • Work collaboratively with the Interdisciplinary Team to develop and implement patient care plans.
  • Conduct thorough assessments of patients' physical, emotional, and social needs.
  • Develop and implement individualized care plans that address patients' unique needs and goals.
  • Provide education and support to patients and their families on end-of-life care and related topics.
Requirements
  • Graduate of an accredited nursing school (Associate of Science in Nursing or Bachelor of Science in Nursing preferred).
  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Two years of acute care nursing experience preferred.
  • Commitment to the hospice concept and philosophy of care.
  • Ability to develop skills in palliative care.
  • CPR certification.
  • Computer skills sufficient to properly document services and care.
  • Valid Driver's License with a safe driving record.
