We are seeking a dedicated and compassionate Hospice Nurse to join our team at Ohio's Hospice. As a member of our Interdisciplinary Team, you will provide patient care that maximizes the comfort and health of hospice patients, their families, and other caregivers.
Responsibilities- Provide supportive care, pain and symptom management, preventative care, and personal care to patients and their families.
- Work collaboratively with the Interdisciplinary Team to develop and implement patient care plans.
- Conduct thorough assessments of patients' physical, emotional, and social needs.
- Develop and implement individualized care plans that address patients' unique needs and goals.
- Provide education and support to patients and their families on end-of-life care and related topics.
- Graduate of an accredited nursing school (Associate of Science in Nursing or Bachelor of Science in Nursing preferred).
- Active Registered Nurse license in the State of Ohio without any board actions.
- Two years of acute care nursing experience preferred.
- Commitment to the hospice concept and philosophy of care.
- Ability to develop skills in palliative care.
- CPR certification.
- Computer skills sufficient to properly document services and care.
- Valid Driver's License with a safe driving record.
