Mass General Brigham is seeking a skilled Registered Nurse Navigator to join our team in Boston, Massachusetts.
About the Role:The Registered Nurse Navigator will be responsible for identifying eligible patients for the MGB Home Hospital program and conducting clinical and social eligibility and safety screening.
The successful candidate will collaborate with the Home Hospital Admitting provider, the Home Hospital Case Manager, and the Administrative Coordinator to ensure seamless patient transfers.
This role requires a strong understanding of the MGB Home Hospital program and the ability to present it accurately to patients, families, and care teams.
Key Responsibilities:- Attend daily huddles with the Home Hospital team to gather clinical information and identify patients for transfer
- Interact with the Emergency Department interdisciplinary team to assist patient identification and transfer
- Collaborate with the MGB Home Hospital Admitting Provider to notify patients of eligibility and obtain consent for transfer
- Communicate with the Administrative Coordinator and Administrative Manager regarding patient transfers
- Utilize analytical and critical thinking to make decisions aligned with organizational objectives
- Current RN licensure in the state of Massachusetts
- Minimum 3 years of experience in emergency department, home hospital, or inpatient settings
- Varied experience preferred
This role is based in a hospital environment, working in an emergency department setting.
