Massachusetts General Hospital is seeking a Medical Home Hospital Advanced Practice Provider to join our team. As a key member of our Home Hospital treatment team, you will provide acute-level care to patients in the comfort of their own home.
Key Responsibilities:
- Provide acute-level care to patients in their homes, including evaluating and managing patients, establishing treatment plans, and responding to patient needs.
- Work with the Home Hospital team to identify patients for transfer/admission to the Medical HH service and complete the admission process.
- Establish relationships with inpatient teams to ensure seamless transition of care to the HH Service line.
- Collaborate with multidisciplinary Home Hospital team members and patients' longitudinal care team members to deliver high-quality care.
- Communicate with patients and families about care plan progression and incorporate patient and family values and goals of care.
Requirements:
- At least 3 years of Nurse Practitioner (NP) or Physician Assistant (PA) experience with a minimum of 2 years of Acute Care Inpatient &/or Emergency experience.
- Current Massachusetts Licensure as a NP or PA with a Master's Degree required.
- Advanced Cardiac Life Support (ACLS) certification required.
Work Environment:
12hr shifts 7A-7P, with rotation of weekends (no more than every 3rd weekend) and rotation of major holidays. Part-time, 24-hours/week, with local travel between patient homes within a defined 10-20 mile radius required.
