LPN Continuous Care Specialist

About the Role
We are seeking an experienced LPN to work in a fast-paced, dynamic environment, providing individualized care to patients in their homes. As an LPN, you will work collaboratively with healthcare teams to develop and implement patient care plans, conducting physical assessments, documenting findings, and reporting changes in patient condition. You will also administer medications, perform treatments, and provide emotional support to patients and families.
Responsibilities
* Develop and implement patient care plans, working collaboratively with healthcare teams
* Conduct physical assessments, document findings, and report changes in patient condition
* Administer medications, perform treatments, and provide emotional support to patients and families
* Maintain accurate records, completing progress notes, forms, and charts as required
Requirements
* One year certificate from college or technical school
* One year recent experience as an LPN required
* Active Practical Nurse license in Georgia and Alabama
* Current CPR Certification
* Valid driver's license and auto insurance coverage
Compensation
The estimated salary for this position is $70,000 - $80,000 annually, depending on experience. A $5,000 sign-on incentive is available for full-time employment, paid in three installments over the first year.
