About the Role

We are seeking a skilled and compassionate Home Health Nurse - LPN to join our team. As a key member of our care team, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.

Job Summary:

  • Implement prescribed plans of care for patients with complex medical needs
  • Conduct thorough assessments and coordinate care with healthcare professionals to ensure seamless transitions
  • Administer medications, treatments, and therapies as ordered by physicians
  • Document patient progress and communicate effectively with family members and healthcare teams
  • Maintain accurate records and adhere to regulatory requirements

Requirements:

  • Active, unencumbered license (RN/LPN/LVN) in the state where you will practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred but not required
  • Reliable transportation
