Job Summary:

As a Skilled Nursing Care Specialist with United Hospital District, you will provide holistic skilled nursing care on an intermittent basis to clients in their places of residence. Your primary responsibility will be to ensure continuity of care through case management, including the performance of skilled assessments, interventions, teaching, maintenance, patient advocacy, and interacting with community/agency resources to provide an ongoing safe environment.

Key Responsibilities:
  • Perform skilled assessments and develop individualized care plans to meet the unique needs of each client.
  • Provide skilled interventions, including wound care, medication management, and education to clients and their families.
  • Collaborate with community/agency resources to ensure a seamless continuum of care.
  • Advocate for clients' rights and interests, ensuring their needs are met and their voices are heard.
  • Document all interactions, assessments, and interventions in a timely and accurate manner.

Requirements:
  • Current RN license in Minnesota.
  • Ability to obtain Iowa RN license after hire.
  • Approximately 40 hours per week, benefits-eligible position.
  • Must be able to take call.
