About the Role:

Matrix Medical Network is seeking a skilled Clinical Health Specialist to join our team. As a Clinical Health Specialist, you will conduct comprehensive health assessments in the comfort of patients' homes, reviewing their current health, medical history, medication adherence, social environment, and other risks. This unique approach provides unparalleled insight into patients' overall health and well-being, enabling us to identify potential care gaps and improve quality of care.

Responsibilities:

• Conduct adult/geriatric assessments, medical history, diagnosis, and treatment
• Provide health education and collaborate with primary care physicians on patient care
• Work collaboratively with case managers, social workers, and other healthcare professionals as needed
• Conduct assessments in various settings, including home visits, skilled facilities, and telehealth visits

Requirements:

• Master's degree in a relevant field or equivalent experience
• Current RN and NP or PA licensure in the state of practice, including prescription authority
• Board certification in a Matrix-approved specialty
• 1 year of experience as a Nurse Practitioner or Physician Assistant
• Strong computer skills and familiarity with EMR software
• Excellent verbal and written communication skills
