We are seeking a highly skilled Clinical Pharmacist to join our team at Advocate Aurora Health. As a Clinical Pharmacist, you will play a critical role in providing exceptional patient care and contributing to the success of our pharmacy team.
Key Responsibilities- Develop and maintain expertise in clinical practice, including appropriate drug therapy for patients and prevention/resolution of drug-related problems.
- Evaluate patients for drug-drug, drug-food, drug-lab, and drug-disease interactions, and individualize drug therapy regimens based on patient size, organ functions, and disease states.
- Facilitate medication-related continuity of care at admission, transfer, and discharge.
- Review and accurately interpret medication orders, and develop competency in dispensing and compounding medications consistent with good pharmaceutical practice.
- Ensure formulary adherence, recommend cost-effective therapies to providers, and minimize waste in daily practice.
- Actively support procedures and initiatives to prevent medication events, and report events through appropriate channels.
- Orient and train new pharmacy team members, and provide direction to pharmacy technicians to ensure smooth workflow and service standards.
- Pharmacist license issued by the state in which the team member practices.
- Immunization and non-vaccine injectable certifications may be required for patient-facing teammates.
- Bachelor's Degree in Pharmacy.
- No experience required.
