Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Advocate Health. This role will require the successful candidate to provide clinical pharmacy services, optimize medication-related continuity of care, and lead medication safety advancements and performance improvement activities.

Key Responsibilities
  • Provide clinical pharmacy services, including expertise in clinical practice, drug therapy, and prevention/resolution of drug-related problems.
  • Optimize medication-related continuity of care at admission, transfer, and discharge.
  • Lead medication safety advancements and performance improvement activities for pharmacy services.
  • Participate in specialty work groups, including councils, committees, and ad hoc teams.
  • Design and implement drug distribution processes to meet patient care needs.
  • Provide training and education to pharmacy team members, including new hires, students, and residents.
  • Lead pharmacy department training programs and precept resident projects.
  • Contribute to the literature through publications and presentations.
Requirements
  • Pharmacist license issued by the state in which the team member practices.
  • Bachelor's Degree in Pharmacy.
  • Typically requires 5 years of experience in specialty residency training or pharmacy.
  • Specialty certification required (for external hires, certification must be obtained within one year of hire).
  • Demonstrated decision-making, team-building, customer service, and conflict resolution skills.
  • Strong computer skills and excellent communication and organizational skills.
Working Conditions
  • Must be able to stand for long periods of time.
  • Will occasionally lift up to 35 lbs. without assistance.
  • Must have functional speech, vision, and hearing.
  • Will be exposed to chemical hazards.
