The Job

We are seeking a skilled Healthcare Manager to lead our integrated care delivery model, managing programs and clinicians providing primary care to SCAN members. The ideal candidate will have a strong background in nursing, with experience in supervising clinical teams and developing clinical care outcomes.

Key Responsibilities
  • Manage programs and clinicians providing primary care to SCAN members within the integrated care delivery model.
  • Oversee the performance of clinical teams to achieve defined clinical care outcomes.
  • Select, onboard, train, and develop clinicians within the integrated care delivery model.
  • Collaborate with senior management on the resolution of operational and performance issues.
  • Develop and implement APP Best Practice Models to improve clinical care outcomes.
Requirements
  • Master's degree from an accredited school of nursing.
  • California RN, NP, and Furnishing licensure required.
  • 5+ years of recent NP experience required.
  • 3+ years supervisory experience preferred.
  • Demonstrated experience in working with both Medicaid and Medicare.
