Job Summary:

The Guidance Center of Westchester's Mobile Crisis Response Team (MCRT) is seeking a skilled Practitioner to join our dynamic team. As a key member of our team, you will provide professional support to licensed clinicians, working together to deliver person-centered, trauma-informed, culturally competent services to individuals in crisis.

Key Responsibilities:

* Build relationships with clients, leading to the development of life goals and enhanced engagement in recovery.
* Link clients to resources, services, and natural supports to support attainment of recovery goals and reduction of isolation.
* Work with clients to increase positive social connections, develop stronger family connections, and improve life skills, including obtaining necessary resources and empowering clients to increase wellness self-management.
* Document all interactions in the Electronic Medical Record (EMR).
* Promote linkage to services by assisting clients with appointment coordination, providing reminders, and escorting or driving to appointments as needed.
* Triage calls, assess clinical responses, and provide support to clients in crisis.

Requirements:

* Bachelor's degree in a Human Services field required.
* 1-3 years of experience in a Human Service-related field and/or lived experience as a consumer of behavioral health services or experience with Mental Health population.
* Demonstrated peer support or mental health experience.
