At Concho Hearts Hospice, our mission is to provide compassionate care to patients in the comfort of their own homes. Our team of dedicated professionals, including nurses, aides, chaplains, social workers, and volunteers, work together to provide symptom management, patient and family support, and emotional care during the end-of-life journey. We are locally owned and operated, ensuring that our patients receive personalized care in a timely manner.

Job Summary:

The primary function of this role is to administer nursing care to patients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient, family, and referring agency, and assume responsibility for coordinating care.

Responsibilities:
  • Develop and adjust patient care plans as needed.
  • Provide loved ones and primary caregivers with patient care information, end-of-life education, and emotional support.
  • Maintain and update medical charts, patient files, and health records.
  • Participate in on-call rotations for weekdays and weekends.
Job Qualifications:
  • Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Licensure: Current license as a registered nurse (RN) and/or by the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC), current Driver's License.
  • Experience: Two years of experience as a Registered Nurse in a clinical care setting, with hospice experience preferred.
  • Skills: Nursing skills as defined by generally accepted standards of practice, good interpersonal skills, and specialized skills in palliation and end-of-life care.
  • Physical and Mental Effort: Prolonged standing and walking required, with the ability to lift up to 50 lbs and move patients.
